Complete Rules & Regulations:
How to Enter: Complete the raffle ticket request form, indicating the number of raffle tickets requested and mail it, along with your payment of $100.00 for each raffle ticket to: DPVA 109 N. Main Street, Suite 200, Dayton, Ohio 45402. For raffle ticket request forms received by the DPVA (DPO) office on or before May 20th, 2008 raffle ticket receipts and numbers will be emailed/mailed to the address on the response form (email is the preferred method of response, please indicate if you will need a response by US Mail). After May 20th, 2008 receipts and raffle numbers will ONLY be emailed. Raffle tickets may be purchased until all are sold or until 8:00pm June 1, 2008. Only 1000 tickets will be available for purchase. Raffle ticket requests will be honored on a first come-first served basis. Mail in payment may be made by personal check, money order or credit card (Master Card or Visa) payable to DPVA. In addition to mail in order, credit card orders can be accepted by email to tshade@daytonphilharmonic.com or by phone 937.224.3521 ext. 134. Ticket sales are final.
Where to get Ticket Request Form: Forms can be downloaded at www.dpva.org, Lexus of Dayton, Dayton Philharmonic Orchestra concerts, DPVA volunteer, DPO office at Schuster Performing Arts Center, 109 N. Main Street, Suite 200, Dayton, Ohio or by calling 937.224.3521 ext. 134. No illegible, incomplete, forged or altered request forms will be accepted. DPVA is not responsible for late, lost, damaged, misdirected or postage due mail. If payment is sent with request form and information is missing, every attempt of the DPVA representative will be made to complete the transaction before monies are processed.
Prize: The winner selected at random with all eligible entries will be able to choose between a Lexus RX 400 Hybrid or a Lexus GS 350 AWD. If the car is accepted by Grand Prize winner, s/he must present evidence of current driver’s license at time of delivery and must take delivery at Lexus of Dayton located at 8111 Yankee Street, Centerville, Ohio. Title, registration, insurance, taxes and all other fees not specified herein are the sole responsibility of the winner. In the event that less than 650 tickets are sold, the prize becomes a cash prize (50/50 winner receives 50% of proceeds and DPVA receives 50% of proceeds).
Winner: Prize winner will be determined from all eligible raffle tickets purchased in a random drawing to be conducted on Sunday, June 1, 2008 following the 75th Anniversary DPO Concert. Odds of winning are 1 in 1000 or better. Ticket holders need NOT be present to win. Winner will be notified by telephone/or mail/or email if not in attendance at the drawing. The winner must accept prize within 14 days and provide social security number and will be required to sign an affidavit of eligibility and release. Prize will be awarded to alternate winner if affidavit is not returned within 14 days after prize notification or if prize notification letter is returned as undeliverable. Winner agrees to use of his/her name, likeness and/or prize information for promotional purposes in any medium without compensation to the extent permitted by law.
Minimum Ticket Sale: If, for whatever reason, less than 650 tickets are sold by the draw date of 8pm June 1, 2008, the prize becomes a cash prize of 50% of ticket sales rather than the Lexus automobile.
Eligibility: Raffle open to any individual legal resident of the United States and 21 years or older who is a licensed driver. Employees of the Dayton Philharmonic Orchestra and volunteers of the Dayton Philharmonic Volunteer Association are eligible to participate. This offer is void where prohibited or restricted by law and is subject to applicable federal, state and local regulations. Taxes and fees are the sole responsibility of the prize winner.
Winner List: The name of the winner will be available online at www.dpva.org after June 15, 2008.
Miscellaneous: By participating in this raffle, each entrant accepts the conditions stated in these official rules, agrees to be bound by the decisions of the Dayton Philharmonic Volunteer Association Board of Directors, and warrants that s/he is eligible to participate in this raffle. By accepting prize, winner agrees to release Dayton Philharmonic Volunteer Association or the Dayton Philharmonic Orchestra Association and their respective directors, trustees, employees, officers, agents, sponsors, advisors, and vendors including without limitation, their advertising and promotion agencies, form any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of their prize or participation in any prize-related activities. Ticket cost is not tax deductible.
